Ultimate Home Health Care’s Palliative Care Program expertly brings together our palliative care team – made up of skilled in-home clinicians from different disciplines. Ultimate’s Director of Nursing, certified in Palliative Care by the California State University’s Shiley Institute, works hand-in-hand with Nurse Practitioners, Physicians, and other members of our clinical team to provide unmatched care. The program uses the latest physician-supervised processes in order to manage symptoms of chronic disease in a supportive methodology that teaches patients and their families the skills needed to live with chronic symptoms.

Palliative Care – What you need to know

When a loved one is seriously ill, it changes all parts of your life. You might feel powerless or alone. It may be hard for you to enjoy life. Where do you turn? Palliative care can help.
Palliative care is focused on comfort and compassion. It also adapts to the special needs of each person. In fact, “Palliate” means relief – which accurately describes this program’s goal of relieving disease symptoms.

This type of care treats the whole person – physically, socially, spiritually, and emotionally – by using an interdisciplinary team. This care team is made up of physicians, nurse practitioners, nurses, social workers, chaplains, and of course the patients and their family members.

Eligibility:
Palliative care is for anyone who is seriously ill. It is care that can be provided at any age and at any stage of the disease. It can also be given along with curative treatment.

Payment:
Some insurance plans cover all or part of palliative care treatments. Medicare and Medicaid may also cover the costs.
