Ultimate Home Health Care’s Palliative Care Program brings together a skilled team of in-home clinicians from various disciplines. Our Director of Nursing, certified in Palliative Care by California State University’s Shiley Institute, collaborates closely with nurse practitioners, physicians, and other clinical team members to deliver exceptional care. This program employs the latest physician-supervised processes to manage chronic disease symptoms, teaching patients
and their families the skills needed to live with chronic conditions.

We offer comprehensive services that address the needs of both patients and caregivers, wherever the patient calls home—whether in a community, a facility,
or their own residence.

Palliative Care –
What you need to know.

When a loved one is seriously ill, it impacts every aspect of your life. You might feel powerless or alone, making it difficult to enjoy life. Where do you turn? Palliative care can help. Focused on comfort and compassion, palliative care adapts to each person's unique needs. The term "palliate" means relief, which perfectly describes the program’s goal of alleviating disease symptoms.

This type of care addresses the whole person—physically, socially, spiritually, and emotionally—using an interdisciplinary team. This team includes physicians, nurse practitioners, nurses, social workers, chaplains, and, importantly, the patients and their family members.

Eligibility:
Palliative care is for anyone who is seriously ill. It is care that can be provided at any age and at any stage of the disease. It can also be given along with curative treatment.

Payment:
Some insurance plans cover all or part of palliative care treatments. Medicare and Medicaid may also cover the costs.
